Variable day(s)/hrs; M-F, 8a-4:30p. Provide evaluations and develop treatment plans for patients in their home/residence.  Coordinate care between team members, facilities and/or physicians. Home Health experience preferred but not required.  Graduate of an accredited Physical Therapy program. Licensed as a PT in Nebraska, drivers license, auto insurance and BLS.  Able to drive own vehicle at times.
