MDS/Discharge Coordinator

16 hrs/wk; 8:30a- 5p. 2 day/wk. Accountable for swing bed care plans to effectively meet patient needs, manage the length of stay, and promote efficient utilization of
resources. Facilitation of certification and payor authorization processes. Facilitation of collaborative management of patient care across the continuum, intervening as
necessary to remove barriers to timely and efficient care delivery and reimbursement and serving as the clinical resources for coders ensuring that documentation accurately
reflects severity of illness and intensity of service. Implements current knowledge and interpretation of medical standards, Medicare Conditions of Participation, initiation of
initial and concurrent reviews, and procedures. Coordinating patient’s transitions of care through effective communication, engaging patient and family as partners of care.
Current NE RN, BSN license with experience and understanding of MDS and RAI regulations. MDS experience preferred
